Three simple steps — from referral to specialist assessment.
Speak with your GP and ask for a referral to Telecare Geriatric Services. Your GP can send it directly via email or fax, or you can upload it yourself through our website. The referral should include the patient's presenting concerns, relevant medical history, and current medications. Referrals are valid for 12 months.
Our team reviews your referral and contacts you to schedule a convenient telehealth appointment. Our administration team will review it within 2 business days and contact you by SMS, phone, or email to confirm your appointment. We aim to connect you within approximately 4 weeks of receiving your referral.
You'll meet with one of our specialist geriatricians or geriatric psychiatrists via a secure telehealth video call. A detailed report is then sent to your referring GP, including clinical findings and recommendations to guide your ongoing care. Family members or carers are welcome to attend and participate.
Conducts your full telehealth assessment and prepares a detailed clinical report with findings and recommendations, sent directly to your referring GP.
Your GP provides the referral and remains the central coordinator of your ongoing care. They receive the specialist report and use it to guide further management, prescribing, and follow-up.
Telecare's telehealth model helps reduce barriers to specialist care, particularly for patients living in rural, regional, or mobility-limited situations. All appointments are conducted securely online and can be attended from home, an aged care facility, or supported living environment.
